Reta (retatrutide) is not yet proven stronger than Mounjaro (tirzepatide), nor Foundayo (orforglipron) better than the Wegovy pill: no head-to-head weight-loss trial of either pair has reported results (ClinicalTrials.gov). In separate trials, average weight change was −25.0% at 80 weeks on retatrutide 12 mg (Jastreboff 2026) and −20.9% at 72 weeks on tirzepatide 15 mg (Jastreboff 2022). Of the four, only tirzepatide is registered in Singapore; individual results vary. GetLean Clinic does not prescribe or supply retatrutide or Foundayo; it supplies HSA-registered GLP-1 medications that Dr Quek prescribes.
For tirzepatide and semaglutide injection, registered in Singapore for weight management:
Availability in Singapore. Both medicines are registered with Singapore's Health Sciences Authority, and their registered indications include weight management. Which medicine suits a particular person, if any does, is decided by the doctor who assesses them.
For oral semaglutide at 3, 7 and 14 mg, the GLP-1 tablet registered in Singapore:
Availability in Singapore. This medicine is registered with Singapore's Health Sciences Authority for type 2 diabetes only. It has no registered weight-management indication here, so prescribing it for weight is off-label: a decision only a doctor who has assessed you can make, and one they may decide against. Which medicine suits a particular person, if any does, is decided by the doctor who assesses them.
For the semaglutide 25 mg tablet and orforglipron, approved abroad:
Availability in Singapore. This medicine is not available in Singapore. It is not registered with the Health Sciences Authority, so no doctor here can prescribe it and there is no legitimate local supply. Anything offered under this name in Singapore is coming from outside the regulated system, and what that actually means is set out here.
For retatrutide, CagriSema and survodutide, in clinical trials:
Availability in Singapore. None of these three is registered with Singapore's Health Sciences Authority, so none can be prescribed or supplied here (HSA register), and none has an application approved by the US Food and Drug Administration (Drugs@FDA). Each is at a different stage. Retatrutide is in phase 3 trials, and Lilly plans to apply for US approval in the first quarter of 2027 (Lilly, July 2026); outside its trials, Lilly releases it only through the pre-approval expanded access set out below. Novo Nordisk applied for US approval of CagriSema in December 2025 (Novo, December 2025); until a regulator approves it, it is given only in clinical trials. Survodutide is in phase 3 trials and is given only to people taking part in them (le Roux 2026). Retatrutide and orforglipron are not registered in Singapore, and GetLean does not prescribe or supply them. GetLean does prescribe and supply HSA-registered GLP-1 medications, after a consultation with Dr Quek. Anything offered for sale under these names is not what the trials studied.
Retatrutide has one exception: Lilly's registry entry for pre-approval expanded access, first posted on 5 June 2026, lists it as available to a small group of adults with severe obesity who cannot join a trial, on their own doctor's request (ClinicalTrials.gov NCT07629401). No application for retatrutide, cagrilintide or survodutide was found on the US FDA's database on 24 October 2026 (Drugs@FDA).
Education only. This page is general education about medicines, most of which are not registered in Singapore. It is not an advertisement or an offer. GetLean does not prescribe or supply any medicine on this page that is not registered in Singapore, and nothing here recommends using one.
Estimand: the rule a trial uses to count its result. A treatment-regimen or treatment-policy estimand counts everyone who was randomised, whether or not they kept taking the medicine. An efficacy estimand estimates the result as if everyone had stayed on treatment. In TRIUMPH-1, the efficacy figure on 12 mg was −28.3% and the treatment-regimen figure −25.0% (Lilly, May 2026). Every trial figure on this page counts everyone randomised unless it says otherwise.
Is Reta (retatrutide) stronger than Mounjaro (tirzepatide)?
No published trial answers that. Retatrutide and tirzepatide have each been tested against placebo, in separate trials, and the trial built to compare them, TRIUMPH-5, has not reported. Its registry entry describes a phase 3, double-blind trial in an estimated 800 adults with obesity, measuring weight change at week 80, with primary completion estimated for November 2026; no results were posted when it was read on 24 October 2026 (ClinicalTrials.gov).
The two placebo-controlled trials, side by side:
| TRIUMPH-1 (retatrutide) | SURMOUNT-1 (tirzepatide) | |
|---|---|---|
| Who took part | 2,339 adults with obesity, without diabetes | 2,539 adults with obesity, or overweight with at least one weight-related complication, without diabetes |
| Average starting weight | 112.7 kg (Lilly's topline announcement) | 104.8 kg |
| Length | 80 weeks | 72 weeks |
| Weekly doses | 4 mg, 9 mg, 12 mg | 5 mg, 10 mg, 15 mg |
| How the result was counted | Treatment-regimen estimand, everyone randomised | Treatment-regimen estimand, everyone randomised |
| Average weight change | −17.6%, −23.7%, −25.0% | −15.0%, −19.5%, −20.9% |
| Placebo | −3.9% | −3.1% |
| Stopped because of side effects | 4.1%, 6.9%, 11.3%, against 4.9% on placebo (topline announcement) | 4.3%, 7.1%, 6.2%, against 2.6% on placebo |
| Funded by | Eli Lilly | Eli Lilly |
Figures by dose, lowest to highest. TRIUMPH-1: Jastreboff 2026, with the starting weight and stopping rates from Lilly, May 2026, a topline announcement. SURMOUNT-1: Jastreboff 2022. Individual results vary and are not guaranteed.
Why the columns are not a ranking. The trials ran for different lengths of time, in people who started at different weights, with different dose steps. Setting −25.0% beside −20.9% and reading the gap as a difference between the medicines treats two separate measurements as one. TRIUMPH-5 is the trial designed to make that comparison. Is Reta stronger than Mounjaro? goes through both medicines' trials in more detail, those in type 2 diabetes included.
Side effects. The most common side effects in both trials were digestive (Jastreboff 2026; Jastreboff 2022). In Lilly's topline announcement of TRIUMPH-1, nausea was reported by 42.4% on 12 mg against 14.8% on placebo, and dysesthesia, an unusual skin sensation, by 5.1%, 12.3% and 12.5% across the three doses against 0.9% on placebo; Lilly says most cases were mild to moderate and resolved during treatment (Lilly, May 2026). The retatrutide side-effects page sets out each trial's figures, and Mounjaro side effects those for tirzepatide.
In Singapore, tirzepatide is registered, including for weight management (Singapore NDF); retatrutide is not (HSA register). The Mounjaro page covers the registered medicine. Nothing here suggests changing from one medicine to another, and a prescribed medicine should never be stopped or changed on one's own.
Foundayo (orforglipron) vs the Wegovy pill: which works better?
No weight-loss trial comparing the two was found, so neither can be called the better one. Each was tested against placebo in its own trial: ATTAIN-1 for orforglipron and OASIS 4 for the semaglutide 25 mg tablet.
| ATTAIN-1 (orforglipron) | OASIS 4 (oral semaglutide 25 mg) | |
|---|---|---|
| Who took part | 3,127 adults with obesity, without diabetes | 307 adults with obesity, or overweight with at least one obesity-related complication, without diabetes |
| Length | 72 weeks | 64 weeks to the main result |
| Daily doses | 6 mg, 12 mg, 36 mg | 25 mg |
| How the result was counted | Treatment-regimen estimand, everyone randomised | All randomised participants (intention to treat) |
| Average weight change | −7.5%, −8.4%, −11.2% | −13.6% |
| Placebo | −2.1% | −2.2% (the US label's analysis gives −2.4%) |
| Stopped because of side effects | 5.3% to 10.3%, against 2.7% on placebo | 6.9%, against 5.9% on placebo (US label) |
| Funded by | Eli Lilly | Novo Nordisk |
ATTAIN-1: Wharton 2025. OASIS 4: the weight figures from Wharton 2025; the counting rule, the label's placebo figure and the stopping rate from the US label. Individual results vary and are not guaranteed.
Why the columns are not a ranking. ATTAIN-1 was ten times the size of OASIS 4, ran eight weeks longer, and tested three doses against one. Novo Nordisk's announcements quote 16.6% for OASIS 4, which is the result when treatment was adhered to, a different count from the −13.6% above (Novo Nordisk). Foundayo vs Wegovy pill: which works better? goes through both trials, the indirect comparison and the side effects in more detail.
A trial of the two molecules together. ACHIEVE-3 compared orforglipron with oral semaglutide, open-label, in 1,698 adults with type 2 diabetes for 52 weeks. It used the 7 mg and 14 mg diabetes doses of oral semaglutide rather than the 25 mg tablet, and its main measure was HbA1c, a blood-sugar measure, not weight. Orforglipron met its goal on HbA1c, and stomach and gut side effects, stopping because of side effects and the rise in pulse rate were all higher on orforglipron (Rosenstock 2026). Foundayo side effects sets out orforglipron's side effects and stopping rates.
How each is taken. Under its UK authorisation, orforglipron is a tablet taken once a day, at any time of day, with no food or water restrictions (MHRA). The semaglutide 25 mg tablet is taken once a day, swallowed whole on an empty stomach after fasting for at least 8 hours, with a sip of water, and nothing to eat or drink for at least 30 minutes afterwards; eating or drinking sooner lowers its absorption (MHRA).
In Singapore, neither tablet is registered (HSA register; HSA approvals). The GLP-1 tablet that is registered here is oral semaglutide at 3, 7 and 14 mg, sold as Rybelsus, for type 2 diabetes only, so prescribing it for weight is off-label (Singapore NDF). The Wegovy pill page and GLP-1 without an injection cover the tablets in more detail.
Reta (retatrutide) vs Ozempic and Wegovy (semaglutide): how do they differ?
Retatrutide acts at three hormone receptors and semaglutide at one, the GLP-1 receptor, and they have been tested in separate trials (Jastreboff 2026; MHRA). Semaglutide is the medicine in both Wegovy and Ozempic, and Wegovy vs Ozempic sets out how the two differ. In Singapore, Wegovy is registered for weight management and Ozempic for type 2 diabetes only (Singapore NDF; Singapore NDF).
In STEP 1, 1,961 adults with obesity, or overweight with a weight-related condition, and without diabetes took semaglutide 2.4 mg weekly or placebo for 68 weeks. Counting everyone, whether or not they stopped treatment, average weight change was −14.9% against −2.4% (Wilding 2021). TRIUMPH-1's figures are in the first table above. The two trials differ in length (68 and 80 weeks) and in who took part, so the figures sit side by side rather than in a ranking. Individual results vary and are not guaranteed.
Reta (retatrutide) or Foundayo (orforglipron): injection or tablet?
Retatrutide is a weekly injection under the skin and orforglipron a daily tablet, and neither is registered in Singapore (Jastreboff 2026; MHRA; HSA register). Orforglipron is approved in the United States and the United Kingdom (Drugs@FDA; MHRA). HSA said on 25 September 2026 that no regulatory authority had approved retatrutide as a medicine (HSA 2026).
In their separate trials, average weight change was −25.0% at 80 weeks on retatrutide 12 mg in TRIUMPH-1 and −11.2% at 72 weeks on the highest orforglipron dose in ATTAIN-1, both counting everyone randomised (Jastreboff 2026; Wharton 2025). They are separate trials with different lengths and different people, and individual results vary and are not guaranteed.
Which will reach Singapore first, retatrutide or Foundayo?
Neither is registered in Singapore or has a Singapore date yet: no date can be given for Foundayo (orforglipron) (HSA register; HSA approvals), and no date has been set for retatrutide, whose announced timetable names the United States (Lilly, July 2026). Foundayo is already approved in the United States and the United Kingdom (Drugs@FDA; MHRA), whereas HSA said in September that no regulatory authority had approved retatrutide (HSA 2026), and an approval abroad does not register a medicine here. GetLean does prescribe and supply HSA-registered GLP-1 medications, after a consultation with Dr Quek. Book a consultation; Dr Quek decides whether a registered medicine suits you.
Foundayo (orforglipron) vs Mounjaro (tirzepatide): tablet or injection?
Orforglipron is a daily tablet that acts at one receptor, GLP-1; tirzepatide is a weekly injection that acts at two, GIP and GLP-1 (MHRA; Jastreboff 2022). Tirzepatide is registered in Singapore, including for weight management; orforglipron is not (Singapore NDF; HSA register).
In separate 72-week trials, average weight change was −11.2% on the highest orforglipron dose in ATTAIN-1 and −20.9% on tirzepatide 15 mg in SURMOUNT-1, both on the treatment-regimen estimand (Wharton 2025; Jastreboff 2022). Their participants and dose steps differ, so these are not a head-to-head result. Individual results vary and are not guaranteed.
What does a pooled analysis of the trials say?
A 2026 network meta-analysis of 262 trials and 99,791 participants compared the drugs indirectly, each against lifestyle measures alone at one year, rather than in one trial (Nong 2026). With moderate to high certainty, it estimated average weight change of −14.9% for tirzepatide, −14.8% for CagriSema, −10.9% for oral semaglutide, −9.9% for orforglipron and −9.8% for semaglutide injection. Newer agents, retatrutide among them, were estimated at 13.1% to 14.6%, with very low to low certainty. Stopping because of side effects was among the highest with orforglipron, CagriSema and oral semaglutide, and the authors found that larger benefits generally came with greater harms and more people stopping. These are pooled averages, not a head-to-head trial, and individual results vary.
CagriSema vs Reta (retatrutide): how do they differ?
CagriSema is a fixed combination of two medicines, the GLP-1 receptor agonist semaglutide and cagrilintide, a long-acting analogue of the hormone amylin, given as a weekly injection under the skin; retatrutide is one molecule acting at three receptors (Novo Nordisk, December 2025; Jastreboff 2026). Novo Nordisk applied for US approval of CagriSema in December 2025 (Novo Nordisk, December 2025). Neither is registered in Singapore (HSA register), and HSA said on 25 September 2026 that no regulatory authority had approved retatrutide or cagrilintide as a medicine (HSA 2026).
In REDEFINE 1, 3,417 adults with obesity, or overweight with an obesity-related complication, and without diabetes were randomised for 68 weeks. On the treatment-policy estimand, average weight change was −20.4% on CagriSema against −3.0% on placebo, and stomach and gut side effects affected 79.6% against 39.9%, mainly short-lived and mild to moderate (Garvey 2025). TRIUMPH-1's figures are in the first table. Separate trials, so not a ranking; individual results vary and are not guaranteed. The CagriSema page covers its trials.
Survodutide vs Reta (retatrutide): how do they differ?
Survodutide acts at two receptors, glucagon and GLP-1; retatrutide acts at those two and GIP. Both are investigational weekly injections, and neither is registered in Singapore (le Roux 2026; Jastreboff 2026; HSA register).
In SYNCHRONIZE-1, 725 adults with obesity, or overweight with an obesity-related complication, and without diabetes took survodutide or placebo for 76 weeks. On the treatment-regimen estimand, average weight change was −12.2% on 3.6 mg and −13.0% on 6.0 mg against −5.4% on placebo (le Roux 2026). Tirzepatide is also a dual agonist, of GIP and GLP-1 rather than glucagon and GLP-1, and its SURMOUNT-1 figures are in the first table. Separate trials again, and individual results vary and are not guaranteed.
What are single, dual and triple agonists?
The terms count how many hormone receptors one molecule acts at.
- Single (GLP-1): semaglutide, as an injection or a tablet, and orforglipron act at the GLP-1 receptor, mimicking a hormone the body releases after eating (MHRA; MHRA).
- Dual: tirzepatide acts at the GIP and GLP-1 receptors (Jastreboff 2022). Survodutide, still investigational, acts at the glucagon and GLP-1 receptors (le Roux 2026).
- Triple: retatrutide, still investigational, acts at the GIP, GLP-1 and glucagon receptors (Jastreboff 2026).
CagriSema is a combination of two molecules rather than one molecule with several targets: semaglutide with cagrilintide (Garvey 2025). The count describes how a medicine is built. What each one does is what its trials measured, set out above.
Where does each medicine stand in Singapore?

| Semaglutide 2.4 mg (Wegovy) | Weekly injection | Registered for weight management (Singapore NDF) |
| Semaglutide (Ozempic) | Weekly injection | Registered for type 2 diabetes only (Singapore NDF) |
| Oral semaglutide 3, 7 and 14 mg (Rybelsus) | Daily tablet | Registered for type 2 diabetes only; prescribing it for weight is off-label (Singapore NDF) |

| Retatrutide | Weekly injection | Not registered; investigational (HSA 2026) |
| CagriSema (semaglutide with cagrilintide) | Weekly injection | Not registered; investigational (HSA 2026) |
| Survodutide | Weekly injection | Not registered; investigational (le Roux 2026) |
Checked on 24 October 2026. The full register download (5,511 records, approvals up to 8 July 2026) contains no product with retatrutide, orforglipron, cagrilintide or survodutide, and no semaglutide tablet other than the 3, 7 and 14 mg strengths (HSA register). HSA's listing of approvals, last updated on 21 October 2026, has none of them among approvals to September 2026 (HSA approvals). An approval abroad does not register a medicine here; that is a separate decision by the Health Sciences Authority.
Retatrutide and orforglipron are not registered in Singapore, and GetLean does not prescribe or supply them. GetLean does prescribe and supply HSA-registered GLP-1 medications, after a consultation with Dr Quek.
Common questions
Is Reta stronger than Mounjaro?
No published trial shows that. Retatrutide (Reta) and tirzepatide (Mounjaro) have been tested against placebo in separate trials, and the trial comparing them directly, TRIUMPH-5, had posted no results on 24 October 2026 (ClinicalTrials.gov). In the separate trials, average weight change was −25.0% at 80 weeks on retatrutide 12 mg (Jastreboff 2026) and −20.9% at 72 weeks on tirzepatide 15 mg (Jastreboff 2022). Individual results vary and are not guaranteed.
Which works better, Foundayo or the Wegovy pill?
No weight-loss trial comparing them was found. In separate trials, average weight change was −11.2% at 72 weeks on the highest orforglipron (Foundayo) dose in ATTAIN-1 (Wharton 2025) and −13.6% at 64 weeks on oral semaglutide 25 mg (the Wegovy pill) in OASIS 4 (Wharton 2025). The trials differ in size, length and dosing, so the figures are not a ranking. Neither medicine is registered in Singapore. Individual results vary and are not guaranteed.
What is the difference between a dual and a triple agonist?
The count is the number of hormone receptors one molecule acts at. Tirzepatide is a dual agonist, at the GIP and GLP-1 receptors (Jastreboff 2022); retatrutide is a triple agonist, at the GIP, GLP-1 and glucagon receptors (Jastreboff 2026). Semaglutide and orforglipron act at the GLP-1 receptor alone (MHRA; MHRA).
Are Reta and Foundayo available in Singapore?
No. Retatrutide and orforglipron are not registered in Singapore, and GetLean does not prescribe or supply them. GetLean does prescribe and supply HSA-registered GLP-1 medications, after a consultation with Dr Quek. Neither is on Singapore's register (HSA register). Orforglipron is approved in the United States and the United Kingdom (Drugs@FDA; MHRA); HSA said on 25 September 2026 that no regulatory authority had approved retatrutide (HSA 2026).
Is there a GLP-1 tablet registered in Singapore?
Yes: oral semaglutide, sold as Rybelsus, at 3 mg, 7 mg and 14 mg. It is registered for type 2 diabetes only, so prescribing it for weight is off-label (Singapore NDF; HSA register). Whether any medicine suits a person is the doctor's decision at a consultation.
Is Rybelsus the same as Mounjaro?
No. Rybelsus is oral semaglutide, a daily tablet registered in Singapore for type 2 diabetes only; Mounjaro is tirzepatide, a weekly injection registered here for weight management and type 2 diabetes (Singapore NDF; Singapore NDF). They are different molecules: semaglutide acts at the GLP-1 receptor, and tirzepatide at the GIP and GLP-1 receptors (MHRA; Jastreboff 2022).
What are the different types of weight loss injections?
The injections on this page are of four types: single agonists, acting at the GLP-1 receptor (semaglutide); dual agonists (tirzepatide, at the GIP and GLP-1 receptors, and survodutide, at the glucagon and GLP-1 receptors); a triple agonist (retatrutide, at the GIP, GLP-1 and glucagon receptors); and a combination of two medicines (CagriSema, semaglutide with cagrilintide) (MHRA; Jastreboff 2022; le Roux 2026; Jastreboff 2026; Novo Nordisk, December 2025). Of these, tirzepatide and semaglutide are registered in Singapore for weight management; the others are investigational (Singapore NDF; Singapore NDF; HSA register). Liraglutide (Saxenda), a once-daily injection not covered on this page, is also registered here for weight management (Singapore NDF).
What is the strongest weight loss injection?
No single trial has compared every weight-loss injection, so no trial names a strongest one. A 2026 network meta-analysis compared them indirectly, each against lifestyle measures alone at one year: −14.9% for tirzepatide, −14.8% for CagriSema and −9.8% for semaglutide injection, with moderate to high certainty, and 13.1% to 14.6% for newer agents including retatrutide, with very low to low certainty; larger benefits generally came with more side effects and more people stopping (Nong 2026). Individual results vary and are not guaranteed.
What are the brand names for semaglutide tablets?
In Singapore, the one semaglutide tablet registered is Rybelsus, at 3, 7 and 14 mg, for type 2 diabetes only (HSA register; Singapore NDF). Abroad, the 25 mg weight-management tablet is sold as the Wegovy pill (MHRA), and the US diabetes label also covers semaglutide tablets sold as Ozempic, at 1.5, 4 and 9 mg (US label). Neither of those is registered here.
What are the differences between orforglipron and Wegovy tablets?
They are different medicines: orforglipron is a small-molecule, non-peptide GLP-1 receptor agonist (Wharton 2025), and the Wegovy tablet is semaglutide (MHRA). Under their UK authorisations, orforglipron is taken once a day at any time with no food or water restrictions (MHRA), and the semaglutide tablet on an empty stomach after fasting for at least 8 hours, with nothing to eat or drink for at least 30 minutes afterwards (MHRA). They were tested in separate trials, not against each other, and neither is registered in Singapore (HSA register).
Which is better for weight loss, semaglutide tablets or Ozempic?
No trial on this page compares them directly. Ozempic is semaglutide injection registered in Singapore for type 2 diabetes only; the semaglutide injection registered here for weight management is Wegovy (Singapore NDF; Singapore NDF). In separate trials in adults without diabetes, semaglutide 2.4 mg injection gave −14.9% at 68 weeks in STEP 1 (Wilding 2021) and the 25 mg tablet −13.6% at 64 weeks in OASIS 4 (Wharton 2025). Individual results vary and are not guaranteed.
