To come off Mounjaro, step the dose down with the prescribing doctor rather than stopping overnight, never on your own. In SURMOUNT-4, adults switched from tirzepatide to placebo regained 14.0% of body weight over a year, on average (Aronne 2024), and pooled across six randomised trials, 60% of the weight lost was regained a year after stopping (Budini 2026); individual results vary. Under the GetLean GLP-1 Taper Plan, Dr Quek plans your taper from the first consultation, toward your lowest effective dose and, where clinically appropriate, off the medication.

Those adults had lost 20.9% over 36 weeks on tirzepatide before the switch, and finished 9.9% below their starting weight. In SURMOUNT-MAINTAIN, adults who stepped down to a lower dose of tirzepatide after 60 weeks were still 16.6% below their starting weight a year later (Horn 2026); individual results vary.

What the labels and SURMOUNT-4 report about how tirzepatide clears, what it does while it is in the body, and the first year after it is stopped.

Weeks one to four: the medicine leaves the body

Slowly, because it was designed to last a week.

The European label gives tirzepatide an elimination half-life of approximately five days, and the United States label five to six days in people with overweight or obesity (EU SmPC, US label). Each half-life halves what is left. The European label's pregnancy advice reflects how long that takes: it says to stop tirzepatide at least one month before a planned pregnancy because of the long half-life (EU SmPC).

So the first missed weekly dose does not switch anything off. Its effect tapers on its own over the weeks that follow, which is part of why some people notice little change at first.

Appetite in the weeks after the last dose

The label says tirzepatide reduces calorie intake, an effect "likely mediated by affecting appetite" (US label). As the medicine leaves, that effect fades over the following weeks. That is why the eating pattern and the training habit are built while the medication is still working, rather than after it has gone. Appetite after stopping GLP-1 goes into more detail.

Side effects

Nausea, vomiting and the other stomach side effects are mostly a feature of starting and of dose increases; in SURMOUNT-MAINTAIN, gastrointestinal events mostly occurred during dose escalation (Horn 2026). Neither label nor any trial we found describes what happens to them after stopping, so we will not guess. The withdrawal-symptom question is covered in Mounjaro and Wegovy withdrawal symptoms.

The first year: what SURMOUNT-4 found

When tirzepatide was switched to placebo, much of the weight came back, but not all of it.

SURMOUNT-4 gave adults tirzepatide for 36 weeks, during which they lost 20.9% of body weight on average. At week 36 they were randomised to continue it or to switch to placebo. Over the following year, from week 36 to week 88, the placebo group regained 14.0% of body weight, and it finished still 9.9% below its starting weight (Aronne 2024).

Those are averages in a studied population, and individual results vary. The regain article covers what the averages hide.

Stepping down rather than stopping

The labels give no instruction on tapering when stopping (US label), and every withdrawal trial took the medication away, by stopping it or by switching people to placebo. At GetLean we favour a gradual reduction under a doctor's guidance rather than a cliff-edge stop, which allows appetite to adjust incrementally: Dr Quek steps the dose down at the monthly reviews, towards the lowest effective dose, as the response comes in. Stepping down is a different picture from stopping: in SURMOUNT-MAINTAIN, adults who stepped down to a lower dose of tirzepatide after 60 weeks were still 16.6% below their starting weight a year later (Horn 2026). Mounjaro or Wegovy maintenance dose sets out that trial, and weaning off GLP-1 how a planned taper works.

Can you microdose Mounjaro to come off?

At GetLean, as part of the GetLean GLP-1 Taper Plan set with Dr Quek: microdosing means taking the medicine below its labelled dose or schedule, and the GetLean GLP-1 Taper Plan gets you there in two ways, and often uses both. With lower doses, you come down to the dose you were on before and stay there until your hunger and cravings are at a level you can handle: someone who reached their goal on Mounjaro 15 mg might come down to 10 mg and stay on 10 mg until they are ready. With longer gaps between injections, instead of one injection every 7 days, you might inject every 8, 9 or 10 days, and once you are on the lowest dose (Mounjaro 2.5 mg), injections can move to once every two weeks.

One safety rule: on higher doses (7.5, 10 or 15 mg), if you go more than 10 days without an injection, the taper restarts on a lower dose. Taking a high dose after a long break can cause stomach and gut side effects, including pancreatitis. Speak to us before your next dose. GLP-1 microdosing at GetLean, step by step sets out the whole plan. Book a consultation to plan yours.

How do I come off Mounjaro without gaining weight?

Come off it in steps with your prescribing doctor instead of stopping overnight, and build your eating and training habits while the medicine is still working. Weight can come back after stopping and no result is guaranteed: in SURMOUNT-4, adults switched from tirzepatide to placebo regained 14.0% of body weight over a year, on average (Aronne 2024); individual results vary. Under the GetLean GLP-1 Taper Plan, Dr Quek plans your taper from the first consultation and steps the dose down at monthly reviews toward your lowest effective dose and, where clinically appropriate, off the medication, with daily WhatsApp coaching building the habits alongside. What makes coming off go better

Building what replaces the medicine before the medicine goes. In SURMOUNT-4, tirzepatide was taken away by switching people to placebo. A planned exit is built the other way round: muscle protected with resistance training, an eating pattern that no longer needs effort, and a weighing habit already running before the last dose. While appetite is quiet, the daily WhatsApp coaching builds the eating pattern and the training habit. Designed to end: planning a GLP-1 exit sets out what that plan contains.

Exercise also counts after the medication stops. In a Copenhagen randomised trial that followed adults for a year after treatment ended, those who had combined a GLP-1 medication with supervised exercise were more likely than those who had taken the medication alone to still hold a loss of at least 10% of their starting weight (Jensen 2024). The medication was liraglutide, an earlier GLP-1 medicine, and the initial loss came from a low-calorie diet.

At GetLean, the programme is designed to end: our philosophy is that GLP-1 medication should act as a catalyst — not something to depend on indefinitely. Already on Mounjaro with another doctor? The taper is planned the same way once you move to GetLean: here is how the switch works, step by step.

Individual results vary, and clinical-trial figures describe the populations studied. Never stop or change a prescribed medicine on your own; that is a conversation with the doctor who prescribed it.

What happens after the taper at GetLean?

Finishing the taper does not have to end the support. After the taper: daily coaching and a monthly review, no medication: S$88 a month, if you wish. If the weight starts coming back, they can come back to Dr Quek at any time: a restart begins with a review consultation, and whether to restart the medication is his decision at that review.

Designed to end: a programme whose pace and taper off the medication are planned from the first consult, so the result holds after the medication stops.

What happens when you stop Mounjaro, or tirzepatide?

The medicine leaves your body slowly, and its effect on appetite fades with it over the following weeks. In the main trial, much of the weight came back, but not all of it.

I'm Dr Quek, a Singapore-registered doctor, and the doctor behind the GetLean GLP-1 Taper Plan.

How long does Mounjaro stay in your system after stopping?

It leaves slowly, because it was designed to last a week. Its half-life is about five to six days, so the amount in your body roughly halves every five or six days. The first missed weekly dose doesn't switch anything off: its effect tapers on its own over the weeks that follow, which is part of why some people notice little change at first.

When does appetite come back after stopping Mounjaro?

Over weeks rather than days. The medicine lowers how much you eat, most likely by acting on appetite, so as it leaves your body, that effect fades.

What happens to side effects after you stop?

Nausea, vomiting and the other stomach side effects mostly come when treatment begins and when the dose goes up. Neither the label nor any trial we found describes what happens to them after stopping, so I won't guess.

What did the main trial see in the year after stopping?

In SURMOUNT-4, adults who were switched from tirzepatide to placebo regained fourteen percent of body weight over the next year, on average, and still finished below their starting weight. Those are group averages, not a prediction for any one person.

Does the label say how to come off it?

The labels give no instruction on tapering when stopping, and every withdrawal trial took the medicine away, by stopping it or by switching people to placebo.

If you're on Mounjaro now, don't stop or change it on your own: that's a conversation with the doctor who prescribed it.

If you're considering GLP-1 medication to reach your weight goal, and you'd like to taper off it one day, I invite you to visit getleanclinic.sg to learn about our approach and the GetLean GLP-1 Taper Plan.

Common questions

How long does Mounjaro stay in your system after stopping?

Tirzepatide's elimination half-life is about five days in the EU label and five to six days in the US label, so the amount in the body roughly halves every five or six days (EU SmPC, US label). Because of that long half-life, the EU label says to stop at least one month before a planned pregnancy.

How much weight comes back after stopping Mounjaro?

In SURMOUNT-4, adults who had lost 20.9% over 36 weeks and were then switched to placebo regained 14.0% of body weight over the next year, finishing 9.9% below where they started (Aronne 2024). Individual results vary.

Do you need to taper Mounjaro?

The labels give no taper instruction, and the withdrawal trials took the medication away, by stopping it or by switching people to placebo. At GetLean the dose is stepped down at Dr Quek's monthly reviews rather than stopped overnight, so appetite can adjust step by step. In SURMOUNT-MAINTAIN, adults who stepped down to a lower dose of tirzepatide after 60 weeks were still 16.6% below their starting weight a year later (Horn 2026). Any change is a decision for the prescribing doctor.

When does appetite come back after stopping Mounjaro?

Over weeks rather than days. The label says tirzepatide lowers calorie intake through appetite, and its half-life is about five to six days (US label), so the appetite effect fades as the medicine leaves the body. The eating pattern and the training habit are built while the medication is still working, so they are in place before that effect has gone.

What happens after the taper at GetLean?

A patient can keep the daily WhatsApp coaching and the monthly reviews with Dr Quek by continuing the monthly subscription, if they wish. If the weight starts coming back, they can come back to Dr Quek at any time; a restart begins with a review consultation, and whether to restart the medication is his decision.

How to wean off of Mounjaro?

Gradually and with the prescribing doctor: the dose is stepped down rather than stopped overnight, and never changed on your own. At GetLean that is the GetLean GLP-1 Taper Plan for coming off Mounjaro: Dr Quek plans the taper from the first consultation and steps the dose down at the monthly reviews towards your lowest effective dose and, where clinically appropriate, off the medication. Individual results vary.

Has anyone kept the weight off after stopping Mounjaro?

In the trials, on average, part of the weight stayed off. In SURMOUNT-4, adults who had lost 20.9% over 36 weeks and were then switched to placebo regained 14.0% of body weight over the next year and finished 9.9% below where they started (Aronne 2024). Stepping down is a different picture: in SURMOUNT-MAINTAIN, adults who stepped down to a lower dose of tirzepatide after 60 weeks were still 16.6% below their starting weight a year later (Horn 2026). These are group averages; individual results vary.

Can you microdose Mounjaro to come off?

At GetLean, as part of the GetLean GLP-1 Taper Plan set with Dr Quek: microdosing means taking the medicine below its labelled dose or schedule, by lower doses, longer gaps between injections, or both. Someone who reached their goal on Mounjaro 15 mg might come down to 10 mg and stay there until they are ready; instead of one injection every 7 days, you might inject every 8, 9 or 10 days; and once you are on the lowest dose (Mounjaro 2.5 mg), injections can move to once every two weeks. On higher doses (7.5, 10 or 15 mg), if you go more than 10 days without an injection, the taper restarts on a lower dose. Taking a high dose after a long break can cause stomach and gut side effects, including pancreatitis. Speak to us before your next dose.
