When Wegovy is stopped, semaglutide stays in your body for about five to seven weeks (US label). In the STEP 1 extension, where semaglutide and the trial's lifestyle support stopped together with no taper, about two-thirds of the lost weight came back within a year (Wilding 2022), and in STEP 4 weight rose 6.9% on average in the 48 weeks after a switch to placebo (Rubino 2021); individual results vary. Through the GetLean GLP-1 Taper Plan, Dr Quek plans your exit from your first consultation, where clinically appropriate.

Semaglutide's elimination half-life is about one week (US label). Dr Quek reviews the dose monthly toward the lowest effective dose and, where clinically appropriate, off the medication.

Availability in Singapore. This medicine is registered with Singapore's Health Sciences Authority, and its registered indication includes weight management. Which medicine suits a particular person, if any does, is decided by the doctor who assesses them.

What the labels and the STEP 1 extension report about how semaglutide clears, what it does while it is in the body, and the first year after it is stopped.

Stage What the labels or trial report Source
The first five to seven weeks An elimination half-life of about one week; present in the circulation for about five to seven weeks after the last injectable dose (US label), or approximately seven weeks after the last 2.4 mg dose (EU label) US label, EU SmPC
While the medicine is in the body Semaglutide reduces calorie intake, an effect "likely mediated by affecting appetite" US label
The first year In the STEP 1 extension, adults who had lost 17.3% over 68 weeks stopped semaglutide and the lifestyle programme at once, with no taper; by week 120 they had regained 11.6 percentage points, about two-thirds of the loss, finishing 5.6% below where they started, on average Wilding 2022

The first five to seven weeks: the medicine leaves the body

Semaglutide was built to last a week, so it also takes weeks to clear.

Both labels give an elimination half-life of approximately one week. The US label says semaglutide is present in the circulation for about five to seven weeks after the last injectable dose (US label); the European label says approximately seven weeks after the last 2.4 mg dose (EU SmPC). Both labels tell people planning a pregnancy to stop at least two months beforehand, for the same reason.

So the effect does not end with the last injection. It fades over the following weeks, which is why some people notice little change at first and more later.

Appetite in the weeks after the last dose

The US label says semaglutide reduces calorie intake, and that the effect is "likely mediated by affecting appetite" (US label). As the medicine clears, that effect fades over the following weeks. That is why the eating pattern and the training habit are built while the medication is still working, rather than after it has gone. Appetite after stopping GLP-1 goes into more detail.

Are there withdrawal symptoms when stopping Wegovy?

Not ones the labels recognise, and the evidence for one is a single reporting signal.

US prescribing information has a standard place for that risk: section 9, "Drug Abuse and Dependence". The prescribing information for semaglutide 2.4 mg goes from section 8 to section 10 without one, and the European product information does not use the words withdrawal, dependence or abuse at all; its leaflet says not to stop the medicine without talking to your doctor (US label, EU SmPC). One 2023 analysis of reports to the US Food and Drug Administration's adverse-event system found that, compared with other GLP-1 medicines, the share of semaglutide reports coded as "drug abuse" or "drug withdrawal syndrome" was about four times higher, with no significant difference against an older weight-loss combination, phentermine-topiramate; the authors say the findings need confirming (Chiappini 2023). Adverse-event reports are filed voluntarily and do not show that the medicine caused what was reported, so it is a signal worth knowing about, not evidence of a dependence syndrome.

What stopping does is let the effects wear off. The appetite effect fades as the medicine clears, and in the STEP 1 extension the cardiometabolic improvements seen on treatment moved back towards their starting levels for most measures within a year of stopping (Wilding 2022). So anyone whose blood pressure or blood sugar improved on treatment should keep monitoring them with their doctor as the dose comes down.

The stomach side effects the labels describe are side effects of taking the medicine, and some people stop because of them: in the trials, 6.8% of adults on semaglutide 2.4 mg stopped because of side effects, against 3.2% on placebo, and nausea, vomiting and diarrhoea were the side effects that most often led to stopping (US label). Neither label describes a reaction caused by stopping. Withdrawal symptoms after Mounjaro or Wegovy covers what the online lists claim.

The first year: what the STEP 1 extension found

When semaglutide and the lifestyle support stopped on the same day, with no taper, much of the weight came back, but not all of it.

In the STEP 1 extension, adults who had lost 17.3% of body weight over 68 weeks stopped both semaglutide and the lifestyle programme that went with it, at once and with no taper. By week 120 they had regained 11.6 percentage points, about two-thirds of what they had lost, finishing 5.6% below where they started (Wilding 2022).

Those are averages from a studied population, and individual results vary. Some people held most of their loss; others regained more than the average. How much weight comes back after stopping covers what the averages hide.

Stepping down rather than stopping

Neither label gives an instruction on tapering when stopping, and the withdrawal trials took the medication away, by stopping it or by switching people to placebo. At GetLean we favour a gradual reduction under a doctor's guidance rather than a cliff-edge stop, which allows appetite to adjust incrementally: Dr Quek steps the dose down at the monthly reviews, towards the lowest effective dose, as the response comes in. Mounjaro or Wegovy maintenance dose covers lower doses, and weaning off GLP-1 how a planned taper works.

Can you microdose Wegovy?

At GetLean, as part of the GetLean GLP-1 Taper Plan set with Dr Quek: microdosing means taking the medicine below its labelled dose or schedule. GLP-1 medicines are started at a low dose and stepped up to an effective one, and tapering runs the same steps in reverse: you come down to the dose you were on before and stay there until your hunger and cravings are at a level you can handle. Only then do we step down again.

The GetLean GLP-1 Taper Plan gets you there in two ways, and often uses both; GLP-1 microdosing at GetLean, step by step sets out both ways and the one safety rule. Every GetLean GLP-1 Taper Plan is set with Dr Quek, around your habits, your lifestyle and how confident you feel at each step. Book a consultation to plan yours.

Coming off Ozempic

Ozempic is the same molecule at lower doses, registered in Singapore for type 2 diabetes only (NDF). It has no registered weight-management indication here, so prescribing it for weight is off-label: a decision only a doctor who has assessed you can make, and one they may decide against. The trial on this page used the 2.4 mg weight-management dose, and its figures describe that dose. Ozempic in Singapore sets out how Ozempic and Wegovy differ and what GetLean prescribes.

The US Ozempic label gives an elimination half-life of approximately one week and says semaglutide will be present in the circulation for about five weeks after the last dose (US Ozempic label). The US Ozempic label gives no instruction on tapering when stopping (US Ozempic label).

If Ozempic was prescribed for type 2 diabetes

This page is about the weight side of coming off. If Ozempic was prescribed for diabetes, any decision to stop, reduce or switch it belongs to the doctor managing the diabetes. GetLean is a weight-management programme and does not replace that doctor.

Already on Ozempic with another doctor

If you are already on Ozempic with another doctor, tell us which GLP-1 medication and dose you are currently taking when you book your consult, and bring any recent records to your consultation. Dr Quek confirms whether the programme is suitable for you at your consultation. Nothing is prescribed or dispensed before he has assessed you, and a consultation does not guarantee that a prescription will be issued. While that is arranged, do not stop or change Ozempic on your own; that is a conversation with the doctor who prescribed it.

What makes coming off go better

Building what replaces the medicine before the medicine goes: muscle protected with resistance training, an eating pattern that has stopped needing effort, and a habit of weighing that is already running before the last dose. In the withdrawal trials the medicine was taken away, by stopping it or by switching people to placebo; a planned exit is built the other way round. While appetite is quiet, the daily WhatsApp coaching builds the eating pattern and the training habit. Designed to end: planning a GLP-1 exit sets out what the plan contains.

Exercise also counts after the medication stops. In a Copenhagen randomised trial that followed adults for a year after treatment ended, those who had combined a GLP-1 medication with supervised exercise were more likely than those who had taken the medication alone to still hold a loss of at least 10% of their starting weight (Jensen 2024). The medication was liraglutide, an earlier GLP-1 medicine, and the initial loss came from a low-calorie diet.

At GetLean, the programme is designed to end: our philosophy is that GLP-1 medication should act as a catalyst — not something to depend on indefinitely.

Individual results vary, and clinical-trial figures describe the populations studied. Never stop or change a prescribed medicine on your own; that is a conversation with the doctor who prescribed it.

What happens after the taper at GetLean?

Finishing the taper does not have to end the support. After the taper: daily coaching and a monthly review, no medication: S$88 a month, if you wish. If the weight starts coming back, they can come back to Dr Quek at any time: a restart begins with a review consultation, and whether to restart the medication is his decision at that review.

Designed to end: a programme whose pace and taper off the medication are planned from the first consult, so the result holds after the medication stops.

Common questions

How long does Wegovy stay in your system after stopping?

Semaglutide's elimination half-life is about one week, and the US label says it is present in the circulation for about five to seven weeks after the last injectable dose (US label); the EU label says about seven weeks after the last 2.4 mg dose (EU SmPC). Both say to stop at least two months before a planned pregnancy.

How much weight comes back after stopping Wegovy?

In the STEP 1 extension, adults who had lost 17.3% over 68 weeks and then stopped both semaglutide and the lifestyle programme at once, with no taper, regained 11.6 percentage points by week 120, about two-thirds of the loss, finishing 5.6% below where they started (Wilding 2022). Individual results vary.

Do you need to taper Wegovy?

The labels give no taper instruction, and the withdrawal trials took the medication away, by stopping it or by switching people to placebo. At GetLean the dose is stepped down at Dr Quek's monthly reviews rather than stopped overnight, so appetite can adjust step by step. Any change to a prescribed dose is a decision for the prescribing doctor.

Does the same apply to Ozempic?

Ozempic is the same molecule, semaglutide, at lower doses, and is registered in Singapore for type 2 diabetes only (NDF). The withdrawal trial described here used the 2.4 mg weight-management dose, so its figures describe that dose.

How long does Ozempic stay in your system after stopping?

Semaglutide's elimination half-life is about one week, and the US Ozempic label says it is present in the circulation for about five weeks after the last dose (US Ozempic label). The US label for the 2.4 mg weight-management product says about 5 to 7 weeks after the last 2.4 mg injection (US label).

I am on Ozempic with another doctor. Can GetLean help me plan coming off?

You are welcome to book your consult and tell us which GLP-1 medication and dose you are currently taking. Dr Quek confirms whether the programme is suitable for you at your consultation, and nothing is prescribed or dispensed before he has assessed you. If Ozempic was prescribed for type 2 diabetes, decisions about stopping or changing it belong to the doctor managing the diabetes.

What happens after the taper at GetLean?

A patient can keep the daily WhatsApp coaching and the monthly reviews with Dr Quek by continuing the monthly subscription, if they wish. If the weight starts coming back, they can come back to Dr Quek at any time; a restart begins with a review consultation, and whether to restart the medication is his decision.

Is it safe to quit semaglutide cold turkey?

Stopping it is not known to cause a withdrawal reaction: neither the US nor the European product information for semaglutide 2.4 mg describes withdrawal or dependence, and the European leaflet says not to stop without talking to your doctor (US label, EU SmPC). What stops with it is the medicine's effect. In the STEP 1 extension, where semaglutide and the trial's lifestyle support stopped on the same day with no taper, about two-thirds of the lost weight returned within a year (Wilding 2022). Under the GetLean GLP-1 Taper Plan for coming off Wegovy, Dr Quek steps the dose down at the monthly reviews instead, towards your lowest effective dose and, where clinically appropriate, off the medication. Individual results vary.

Why are people stopping Wegovy?

People stop for more than one reason, and side effects are the one the trials report: 6.8% of adults on semaglutide 2.4 mg stopped because of side effects, against 3.2% on placebo, and nausea, vomiting and diarrhoea were the side effects that most often led to stopping (US label). In routine US practice treatment often ends early: among 125,474 US adults starting one of these medicines, 64.8% of those without type 2 diabetes had stopped within a year, moderate or severe gastrointestinal side effects predicted stopping, and larger weight loss predicted staying on (Rodriguez 2025). At GetLean the exit is planned from the first consultation instead, under the GetLean GLP-1 Taper Plan for coming off Wegovy, towards your lowest effective dose and, where clinically appropriate, off the medication. Individual results vary.

Can I quit Wegovy after 3 months?

None of the withdrawal trials stopped the medication at 3 months: the closest is STEP 4, which switched adults to placebo after a 20-week run-in on semaglutide, and their weight rose by 6.9% on average over the next 48 weeks (Rubino 2021). When to stop is the prescribing doctor's call, made on how you have responded, and never one to make on your own. Under the GetLean GLP-1 Taper Plan for coming off Wegovy, the taper is planned from the first consultation and set at Dr Quek's monthly reviews as the response comes in. Individual results vary.

The GetLean GLP-1 Taper Plan gets you there in two ways, and often uses both; GLP-1 microdosing at GetLean, step by step sets out both ways and the one safety rule.
